CUSTOMER RIGHTS

We believe that all customers receiving services from Ed Medical, Inc. should informed of their rights. Therefore you are entitled to:

  1. Be treated with dignity, courtesy, and respect
  2. Select those who provide your home care services.
  3. Be provided with adequate information so that you can make informed decisions regarding your care.
  4. Receive a timely response form Ed Medical, Inc. when home care equipment is needed or requested.
  5. Be fully informed of all policies, procedures and charges for services and equipment.
  6. Receive an explanation of all forms you are requested to sign.
  7. Receive home care equipment regardless of race, religion, political belief, sex, social status, age, sexual preference or handicap or any other classification protected by law.
  8. Receive proper identification from personnel providing your service.
  9. Participate in all aspects of your care including the right to refuse service within the confines of the law.
  10. Have all your records and all communications, written or oral, treated confidential.
  11. Have access to all health records pertaining to you.
  12. Express dissatisfaction and suggest changes in any service without fear of coercion.
  13. Receive information on Ed Medical, Inc.’s mechanism for receiving, reviewing and resolving your complaints or concerns.
  14. To formulate and have honored an advance directive such as a Living Will or a Durable Power of Attorney for Health Care.
  15. To be involved, as appropriate, in discussions and resolutions of conflicts and ethical issues related to your care.
  16. To have your privacy and property respected at all times.
  17. Be informed of all your responsibilities regarding home medical equipment usage and services.

 
 

CUSTOMER RESPONSIBILITIES

  1. Customer agrees that rental equipment will be used with reasonable care, not altered or modified, and returned in good condition when the need for such equipment no longer exists.
  2. Customer agrees to promptly report any malfunctions or defects in the equipment to Ed Medical, Inc..
  3. Customer agrees to use the equipment for the purposes so indicated and in compliance with the physicians prescription.
  4. Customer agrees to keep the equipment in his/her possession and shall keep the equipment at the address to which it was delivered unless otherwise authorized by Ed Medical, Inc.
  5. Customer agrees to permit authorized company representatives access to all company owned equipment for the purposes of performing service, repair, replacement, or retrieval.
  6. Customer agrees to notify Ed Medical, Inc. of any change in customer insurance, address, physician, etc.
  7. Customer agrees to accept all financial responsibility for home medical equipment furnished by Ed Medical, Inc..
  8. Customer agrees to notify Ed Medial, Inc. of any change in condition or circumstances.
  9. Customer agrees to involve him or herself in the provision of care, as able.
  10. Customer agrees to maintain a safe environment in the home.
  11. Customer agrees to notify Ed Medical, Inc. when they will not be home at the time of a scheduled visit.
  12. Customer agrees to make a conscious effort to comply with all aspects of the plan of service.
Customer agrees to notify physician when feeling ill.
